Instructions for Completing a Voter Registration
Application

When completing the application, pay extra
attention to the shaded lettered boxes. These fields
on
the application must be completed for the registration to be valid. The
other areas on the application are optional however, applicants are
encouraged to provide as much information as possible.

Options for filling out the voter registration
application form:

PRINT the form, fill it out by hand, sign and
mail to the Department of Elections; or
-
Complete the form online, PRINT, sign and mail to
the Department of Elections. (The form has fields that give you the
opportunity to select and type information.)

The form must be sent with an original signature or
mark.
The form will be considered incomplete and the registration will be
invalid without a signature or mark.

Boxes A, B, C, D, E, F, G and H are required to be
completed.

The form will be considered incomplete and the
registration will not be valid if boxes A, B, C, D, E, F, G and H are not completed.

Once the form has been completed, print, sign and
mail to:

Please Note: The
application will not be valid until the completed form with original
signature or mark is received by the Supervisor of Elections Office.
